Hospice Field Registered Nurse (RN)

What you'll do

• Conduct initial and ongoing comprehensive assessments
• apply hospice admission and recertification criteria
• develop, document, and revise plans of care
• educate patients and families about disease processes, self-care, end-of-life care, and nutrition
• determine service scope and frequency
• assess caregiver capacity
• provide clinical direction to hospice aides and LPN/LVNs
• prepare clinical and progress notes
• communicate changes and outcomes to physicians and other personnel
• perform assigned on-call responsibilities.
