What you'll do

• Receive referral requests
• verify eligibility and patient demographics
• contact patients about referrals and appointments when needed
• coordinate with providers, specialty practices, community organizations, and clinical consultants
• schedule and confirm appointments
• maintain referral documentation and data in the EMR
• contribute to data capture and reporting
• support turnaround expectations
• assist with referral process audits
• maintain patient confidentiality.
