Through eligibility screening, needs assessment, individualized support planning, referrals, follow-up, and advocacy, the case manager addresses barriers to healthcare access and service participation.

What you'll do

• Screen clients for initial and annual Ryan White eligibility and maintain recertification records
• assess social, economic, housing, food security, and transportation needs
• develop individualized support plans
• refer clients to community services and follow up on access and outcomes
• provide crisis intervention and advocacy
• share information on HIV prevention and disease management basics
• document assessments and interactions in the EHR according to HIPAA and Ryan White standards
• build relationships with community organizations and service providers. Community outreach and home visits are preferred qualifications
• HIV-specific and cultural competency training must be completed within the first year.

Why this role

Applicants may qualify with a bachelor’s degree in any field, or a high school diploma/GED plus three years of HIV-related services experience. Spanish or Vietnamese bilingual ability is a plus.
