Transitional Care Management Program Manager

The Transitional Care Management Program Manager role is onsite in Salisbury, NC. Compensation is $25–$30 per hour. The position supports SHYAS Health’s care management team and service delivery. The manager supervises clinical and administrative work, monitors care-management quality and engagement, and ensures required assessments and follow-up occur within service timelines. Oversight, staff development, record review, and outcome reporting help address care gaps and maintain service standards.

What you'll do

• Provide oversight, direction, and clinical and administrative supervision to the care management team
• hire, discipline, terminate, and train staff
• monitor service quality and compliance with service definitions
• facilitate case conferences, audit records, and observe team activities
• monitor engagement, assessments, care coordination, health promotion, transitional care, family supports, and referrals
• ensure care gaps are addressed
• monitor acuity-based engagement timeframes
• collect and report outcomes
• represent the agency in internal and external meetings.
