The role supports assessment, crisis response, treatment planning, and linkage to clinical and community resources.

What you'll do

• Complete intake Core Assessments and collateral assessments
• support development of Individual Client Plans with members and the treatment team
• identify strengths, challenges, treatment barriers, and natural supports
• complete required paperwork
• provide crisis intervention and on-the-spot counseling
• coordinate transitions and referrals
• develop initial safety plans
• facilitate groups and provide individual and family therapy.
