What you'll do

• Assess and reassess hospice patients
• provide in-home nursing services, treatments, and preventive procedures
• monitor changes in symptoms or behavior and report unexpected changes
• provide emotional support and teach patients and families about care
• develop and reevaluate care plans with the interdisciplinary group
• follow physician orders
• maintain records and confidentiality
• coordinate care and comply with infection-control policies. Participate in the on-call system and provide coverage when unavailable for assigned duties.
