What you'll do

• Support community activities, appointments, errands, and social opportunities
• teach daily living skills such as cooking, cleaning, budgeting, and shopping
• assist with personal care and medication reminders as authorized
• provide safe transportation
• follow individualized service plans and person-centered goals
• complete service notes and required documentation
• promote independence, dignity, choice, and self-advocacy.
