What you'll do
- Communicate status and participate in person and team/caseload activities
- implement supports for health, safety, and community participation
- provide personal care
- conduct service checks of homes and supplies
- research and communicate community resources
- assist with transportation access and participation in events
- administer medications as delegated by a nurse and accompany people to medical appointments
- monitor household maintenance and purchases
- collaborate with coordinators, staff, families, and community organizations
- attend meetings and trainings
- complete required documentation.
