What you'll do

• Assess patient and caregiver needs and abilities
• participate in developing patient-specific training and care plans and provide training when appropriate
• relay orders and discharge documentation to NELC departments
• serve as a clinical resource to hospital departments
• coordinate with home health agencies
• monitor patient status during hospitalization
• document interactions
• communicate patient requirements and status changes
• participate in care meetings and case conferences
• protect patient confidentiality
• complete therapy-specific reimbursement determination checklists.
