The role supports coordinated home health services through communication with clinical management and timely, accurate documentation.

What you'll do

• Provide one-on-one patient care and patient/family teaching in the home
• conduct patient assessments

• complete timely and accurate documentation
• maintain current RN licensure and CPR certification
• use reliable transportation for field work.
