Hospice Social Worker

The Hospice Social Worker role is onsite in Hudson, WI, with on-the-road work; compensation is $26–$35 per hour. What you'll do

• Provide psychosocial services to terminally ill patients and families
• contact patients, families, and caregivers within 48 hours of admission
• complete psychosocial assessments for the patient chart within 24 hours
• assess and reassess psychosocial and pre-bereavement needs
• implement care-plan social service goals, supportive counseling, problem solving, community referrals, and bereavement care
• participate in weekly interdisciplinary team meetings
• assist with funeral arrangements or counseling if requested.
