What you'll do

• Lead intake, introductory, and supervisory visits
• support customized care plans, client retention, and care quality
• oversee visit tracking and audit compliance documentation
• use internal systems such as WellSky for data and care coordination
• lead caregiver onboarding, orientations, and coaching
• coordinate communication among clients, families, and care coordinators
• proactively resolve service gaps and complex situations.
