What you'll do

• Make inpatient visits to hospice patients in contracted facilities
• consult on pain and symptom management
• provide in-service education to providers
• conduct pre-admission evaluations
• explain hospice services and obtain informed consents
• provide assessment, initial care plans, and interventions
• coordinate discharge planning
• maintain communication with admissions and team directors
• support provider relationships and hospice consultation.
