Job overview

The Hospice PRN Social Worker role is onsite in Snellville, GA, serving hospice patients and families; compensation is $33 per hour.   • Conduct initial and ongoing psychosocial, emotional, spiritual, and bereavement assessments
  • contribute to interdisciplinary care planning
  • provide counseling and short-term crisis intervention
  • advocate for patients and families
  • coordinate community referrals
  • document visits, contacts, and assessments
  • participate in bereavement services and team meetings.
