What you'll do
- Develop written plans of care with the interdisciplinary team
- oversee implementation and evaluate or revise plans to meet patient and family needs
- communicate changes to the team and primary physician
- perform nursing duties within scope of practice and standards of care
- maintain timely electronic point-of-care documentation
- follow hospice policies and regulatory requirements
- provide health care instruction to patients and families
- orient, instruct, and train assigned personnel
- perform admission assessments and admissions when necessary
- travel for patient visits.

Why this role
Requires an associate degree, current California RN license, current CPR/BLS, California driver’s license, and at least one year of acute or pediatric hospice experience. Hospice nurse certification and bilingual ability are preferred.
