• Receive home care service requests and schedule or reschedule Care Partners
• gather case information
• check Care Partner availability
• confirm schedules with clients and Care Partners and verify visits
• maintain schedules and availability records in ContinuLink
• update client information
• perform information audits.

Why this role

High school diploma or equivalent and at least one year of home care, human resources, community service, or supervisory experience are required. Staffing-industry scheduling experience is preferred.
