Care Management and Education Nurse

The Care Management and Education Nurse role is an onsite position based in Whitefield, NH, with regular travel to all four Ammonoosuc Community Health Services locations. Compensation is not specified. The nurse supports clinical staff education, patient safety, quality, infection control, and care management across ACHS sites. The role develops and evaluates staff education and competency processes while serving as a clinical resource for support staff. It also contributes to transitional care management and patient support, helping promote safe, high-quality care and continuity after hospital discharge.

• Conduct training, education, and competency assessments for clinical support staff
• help develop and reinforce clinical standards and workflows
• participate in onboarding and competency validation
• identify learning needs
• contribute to patient safety, quality, and infection control initiatives
• support documentation and process improvement
• serve as a clinical resource
• provide clinic coverage as needed
• educate patients and families
• assist with appointments and referrals
• deliver transitional care management after hospital discharge
• respond to patient and staff needs
• provide clinical guidance.

Why this role

Regular travel to all four ACHS locations is required. Chronic Care Management, remote patient monitoring, patient education visits, and clinic phone triage may be added as staffing allows.
