The role also assesses preferences, documents activity-related care information, and coordinates programming with residents, families, staff, and the community.

What you'll do

• Plan, organize, and implement activity and leisure programming
• complete activity preference and assessment documentation, including applicable MDS and CAA sections
• attend resident care plan meetings
• prepare and post a monthly activity calendar
• identify resident preferences and health issues affecting participation
• develop individualized plans based on needs, strengths, and preferences
• support participation within functional or situational limitations
• plan community activities involving residents, families, staff, and community members
• maintain precautions and limitations information
• encourage group and individual leisure pursuits.

Why this role
