What you'll do

• Ensure newly admitted clients receive social-service intake within 48 hours of assignment
• track intakes and move-outs
• oversee client cases and monthly tracking forms
• coordinate high-risk case conferences with community agencies
• supervise staff
• set and monitor performance targets
• coach staff on client engagement and entitlements
• conduct weekly progress reviews and chart reviews
• respond to critical incidents
• prepare contract reports and statistics
• organize staff training
• facilitate weekly social-service meetings
• review timesheets and prepare performance appraisals.
