What you'll do

• Respond to inbound calls, website inquiries, and referral opportunities
• guide families through care options and service pathways
• coordinate consultations, assessments, and admissions
• build relationships with patients, families, and referral sources
• manage an active pipeline and follow-up
• collaborate with clinical, intake, scheduling, marketing, and leadership teams
• occasionally meet families in homes, hospitals, rehabilitation facilities, and senior living communities.
