Direct Care Worker

The Direct Care Worker role is onsite in Phoenix, AZ, paying $15–$17 per hour. The position supports adults with developmental disabilities through day treatment services. The worker provides individualized habilitative training, supervision, and personal care to help members build or maintain social, communication, and adaptive living skills and participate in their communities. Services follow each member’s Person-Centered Service Plan or Individual Service Plan.

What you'll do

• Implement individualized goals and outcomes
• provide sensory-motor, cognitive, communication, social interaction, and behavioral training
• facilitate community integration
• assist with personal care and feeding as needed
• support therapy carryover when applicable
• maintain visual supervision
• follow health, safety, medication, incident-reporting, infection-control, and emergency procedures
• document attendance, services, progress, incidents, and health changes
• communicate with members, representatives, support coordinators, clinicians, and agency leadership.

Why this role

Requires age 18 or older, professional references, an Arizona Fingerprint Clearance Card, current CPR and First Aid certifications before independent work, and required agency training. Arizona driver’s license is required if transporting members.
