What you'll do

• Coordinate and provide individual and group therapy and biopsychosocial assessment services
• develop person-centered care plans
• document client progress in SOAP notes
• prepare transition notes, discharge plans, and summaries
• participate in clinical treatment team meetings and agency training
• intervene in crisis situations
• support referrals and discharge planning
• maintain confidentiality and comply with professional ethics and clinical standards.
