Travel Physical Therapist (PT) - Home Health - Medford, OR - $2,400 to $2,600 per week. This is an onsite position. AlliedTravelCareers is working with United Health Care Staffing to find a qualified Physical Therapist (PT) in Medford, Oregon. The pay is $2,400 to $2,600 per week for a 13-week contract starting ASAP. This role involves delivering skilled physical therapy services in patients’ homes, evaluating and treating patients with orthopedic and neurologic conditions, and developing and implementing individualized care plans.

What you'll do

• Deliver skilled physical therapy services in patients’ homes.
• Evaluate and treat patients with orthopedic and neurologic conditions.
• Develop and implement individualized care plans.
• Perform patient assessments and track progress.
• Educate patients and caregivers on home exercise programs.
• Complete accurate and timely documentation (OASIS required).
