The Registered Nurse (RN) – Hospice role is a part-time, field-based position serving the greater Dedham, MA area, with care provided in home or facility settings. The nurse provides clinical care and emotional support to hospice patients and families. Through individualized care planning, symptom management, education, medication management, and interdisciplinary collaboration, the role supports comfort-focused care and patient and family needs.

What you'll do

• Conduct patient assessments and develop individualized hospice care plans
• provide skilled nursing care and symptom management in home or facility settings
• educate and support patients and families about disease progression, comfort measures, and hospice philosophy
• collaborate with physicians, social workers, chaplains, and aides
• manage medications and pain and palliative care protocols
• document care in compliance with Medicare and Medicaid hospice regulations
• provide on-call support and respond to urgent needs.
