PRN Registered Nurse Case Manager

When assigned, the nurse manages a patient caseload and helps maintain continuity of care.

What you'll do

• Complete initial and ongoing patient and family assessments
• provide nursing care and develop or revise care plans
• administer prescribed medications and treatments
• counsel and educate patients and families
• assist with activities of daily living
• coordinate assigned caseloads when designated
• maintain clinical notes and communicate changes to physicians
• coordinate with community health contacts and the interdisciplinary group
• participate in on-call duties as defined by policy
• supervise hospice aides when required.
