The Social Worker role is onsite in Peabody, MA, at the Jeffrey & Susan Brudnick Center for Living. Pay is $30 to $63 per hour, based on experience. What you'll do

• Conduct psychosocial assessments and develop individualized care plans
• provide counseling, intervention, and ongoing support to residents and families
• advocate for resident rights and quality of life
• collaborate with the interdisciplinary care team to coordinate services and facilitate care conferences
• and connect residents and families with community resources and post-discharge support.

Why this role
