What you'll do

• Assist patients and families with health-related financial, social, and emotional concerns and coping skills
• provide social work services according to the plan of care
• communicate clinical-status changes to the healthcare team and care center office
• educate patients, families, and team members about interventions and treatment plans
• perform care planning, transfer, and discharge functions
• identify environmental risks and coordinate interventions with the physician and care team.
