What you'll do

  • Conduct comprehensive medical eligibility assessments by telephone, electronically, or through in-person home visits
  • evaluate applicants for State Plan Personal Care, ADvantage Waiver, and occasionally Title 19 nursing home eligibility
  • make recommendations based on assessed needs
  • assist with Adult Protective Services evaluations when requested
  • and review, audit, and analyze nursing and healthcare services. Extensive travel and occasional short-notice home visits are required.
