The role contributes to individualized care by coordinating services, monitoring changing needs, documenting treatment, and working with treatment teams and agents.

• Meet clients and establish rapport
• conduct assessments and screening
• complete case-management assessments
• coordinate client needs through individualized treatment plans
• support daily living skills and community-resource use
• monitor clients in the facility
• encourage family participation
• provide treatment information to the team
• document assessments, treatment, and communications
• work with agents on case planning.
