Psychiatric Rehabilitation Service Coordinator (PRSC)

What you'll do

• Evaluate existing, new, and prospective clients based on needs, limitations, and goals
• address concerns and maintain communication
• build rapport with clients and families
• coordinate care with other departments
• develop care plans
• complete MDS and care-plan input and assessments in a timely manner
• monitor progress and modify treatment plans
• maintain records and report client status
• refer clients to community services.
