Working with Veterans and their multidisciplinary teams, the role supports informed care decisions, service coordination, and continuity of care without providing health care directly.
What you'll do
- Assess Veterans with the interdisciplinary team, family members, and others
- identify strengths, risks, supports, and service needs
- enter timely and complete data in HMIS or equivalent systems and client files
- help Veterans communicate care preferences and goals
- provide education and identify appropriate resources
- advocate for clients and incorporate cultural values into care plans
- monitor progress
- consult with team members
- identify systemic barriers and collaborate on solutions
- coordinate complementary services
- participate in team meetings and case conferences.

Why this role
