Job overview

The Registered Nurse (RN), Home Health role is an onsite position based in Miles City, MT, serving patients through home visits. The hourly pay range is $35–$45. Granite Peak Home Health provides home health services and identifies the Home Health Director as the role’s reporting manager. The nurse evaluates patient needs, develops and updates plans of care, and provides specialized nursing services in the home. The role coordinates care with patients, families, physicians, and other clinicians to support continuity, appropriate treatment, and accurate clinical records.

What you'll do

• Provide services according to the plan of care
• conduct initial and ongoing nursing evaluations
• initiate and revise care plans
• provide specialized and preventive nursing procedures
• prepare clinical notes and care-conference summaries
• coordinate services and communicate changes in patient condition
• counsel patients and families
• process orders and notify physicians
• refer patients to other disciplines as needed
• supervise and teach nursing personnel
• participate in care conferences and quality activities
• complete OASIS assessments and related documentation
• and take assigned on-call duty nights, weekends, and holidays.
