What you'll do

• Conduct prospective-client calls and in-home assessments
• develop, implement, and update written Quality of Life Plans
• deliver individualized and group activities
• coordinate materials and address implementation barriers
• document services and client responses
• communicate with families and elder-care professionals
• support home-care programming and PCA training
• build community relationships and share best practices with the team.
