What you'll do

• Serve as a point of contact among referring physicians, patients, and caregivers
• complete telephonic pre-operative risk assessments
• collaborate on individualized care plans
• connect patients with clinical and community support services
• gather clinical and assessment information for risk stratification
• track and analyze data
• educate patients and caregivers
• assist with educational programs
• coordinate services with multidisciplinary providers.

Why this role
