Home Health Field Registered Nurse

What you'll do

• Schedule patient visits
• complete admission and follow-up assessments, including vital signs and medication reconciliation
• document findings and develop plans of care
• contact physicians for orders and report changes
• supervise and train PCAs and HHAs
• monitor service quality and coordinate care
• complete recertifications, transfers, discharges, and OASIS documentation as appropriate
• counsel patients and families
• maintain records and regulatory compliance.
