This work helps support timely transitions and patient-centered rehabilitation outcomes.

What you'll do

• Guide treatment plans with the interdisciplinary team based on patient needs and preferences
• coordinate tentative discharge and contingency plans
• participate in discharge planning and execution
• facilitate weekly team conferences and treatment-plan modifications
• complete case management documentation
• review reports and payer requirements
• meet with patients and families within 24 hours of admission and complete assessment documentation within 48 hours
• educate patients and families
• arrange resources and referrals
• participate in utilization review and the case management on-call schedule as needed.
