What you'll do

• Help plan and implement the Social Services program in compliance with regulations
• advocate for patient and resident rights and well-being
• conduct psychosocial assessments and develop person-centered care plans
• educate patients, families, and staff about health-care decisions, advance directives, and community resources
• coordinate discharge planning
• provide therapeutic interventions and facilitate groups.
