What you'll do

• Provide prerelease and post-release case management
• travel to prisons, jails, probation and parole offices, and other criminal justice providers to promote and recruit eligible participants
• conduct intake and assessments
• monitor client services and needs
• develop plans for clients releasing to identified counties
• provide information and referrals
• link clients and families to resources through advocacy and discharge planning
• participate in case conferences
• document services according to state regulations
• attend required training.
