What you'll do

• Serve as a liaison among hospitals, assisted living communities, physicians, and other referring providers
• plan and coordinate patient referrals and provider relationships
• secure new customers and coordinate admissions for new and returning patients or residents
• communicate admission updates, bed changes, discharges, tours, meetings, and events to facility leadership
• provide information about facility services to patients, residents, and families
• visit inpatients and provide education to potential referral sources, patients, and families
• develop census
• implement public relations and marketing programs
• attend community events
• conduct facility tours and follow up on inquiries.
