Clinical Liaison — Hospital / Skilled Nursing

The role builds relationships with physicians, hospitals, and other community referral sources to generate patient referrals and support conversion to admissions. It coordinates the referral-to-admission process, monitors sales activity and performance indicators, and helps address barriers to admission.

What you'll do

• Develop strategies to gain patient referrals by establishing and maintaining relationships with physicians and healthcare providers
• generate patient activity and strengthen referral-source relationships
• formulate sales plans and monitor sales processes
• drive referral, conversion, and customer-satisfaction indicators
• conduct one-on-one sales calls and community interactions
• serve as a resource for internal and external customers
• identify new business opportunities
• develop census toward business-plan goals
• assist with referral-to-admission coordination
• represent the organization in community activities
• respond to admission barriers.
