Coordination with clinical, business, and social services teams supports appropriate placement, complete documentation, and the community’s census and occupancy goals.

What you'll do

• Respond to referrals and inquiries
• coordinate admissions from referral through arrival
• gather and review clinical, medical, financial, and admission documentation
• partner with Nursing leadership on placement
• coordinate room assignments and move-in preparation
• conduct community tours and follow up with referral sources
• verify payer sources and insurance coverage
• maintain accurate records and support census reporting and compliance.
