Hospice Community Liaison

Job overview

The Hospice Community Liaison role is onsite in Lebanon, PA, serving the Lebanon region. Base salary is $50,000–$95,000 per year, plus monthly, quarterly, and annual bonuses. Ennoble Care provides mobile primary care, palliative care, and hospice services to patients, including people with chronic conditions and limited mobility. The liaison promotes hospice services and builds community referral relationships to support patient access. The role analyzes service-area opportunities and marketing data, develops and implements marketing plans, and provides community outreach and education.

What you'll do

• Analyze the service area and identify target markets
• visit physician offices, hospitals, assisted living facilities, skilled nursing facilities, and other referral sources
• present agency credentials and obtain patient referrals
• analyze organizational strengths and weaknesses, marketing data, service offerings, and patient relationships
• develop sales and marketing objectives, projections, and plans
• implement and review marketing plans
• attend IDG meetings as appropriate
• assist office staff as needed
• provide community outreach and education.
