The Clinical Admissions Liaison role is onsite in Braintree, MA; salary is $75,000–$85,000 per year. The position supports BaneCare Management’s admissions team and skilled nursing centers. The liaison assesses clinically complex patients before admission to determine whether clinical, fiscal, and regulatory criteria are met. The role also maintains referral relationships and admission and census levels, helping patients and families transition from acute hospitals to post-acute rehabilitation or long-term care.

What you'll do

• Assess clinically complex patients before admission to skilled nursing centers to ensure clinical, fiscal, and regulatory criteria are met. Work with new and existing referral sources
• maintain admission and census levels
• build relationships with hospital providers and skilled nursing centers. Position is primarily in-house, with occasional travel to referral sources and skilled nursing centers. Valid driver’s license, dependable transportation, and auto insurance required. RN/LPN or social work license and prior SNF clinical liaison experience preferred, not required.
