What you'll do

• Monitor residents’ medical needs and communicate with healthcare providers and agencies
• attend person-centered planning meetings
• assist with mobility, transfers, bathing, hygiene, grooming, laundry, meals, and household tasks
• support communication and community activities
• pass medications according to training and physician orders
• transport residents when needed
• respond to emergencies
• complete paperwork and incident reports
