The Psychiatric Nurse Care Coordinator role is onsite at the Suffolk County House of Corrections in Boston, MA. The schedule is Monday through Friday, 40 hours per week. Pay is $55 to $63 per hour, depending on years of licensure and shift. CPS Healthcare provides medical and behavioral health services to communities and correctional facilities. The registered nurse coordinates continuity and safe delivery of psychiatric services by organizing clinical information, medication follow-up, and communication among nursing, mental health, medical, pharmacy, and psychiatric providers. The role supports an organized care workflow while leaving diagnosis, prescribing, and treatment-plan decisions to the psychiatric provider.
What you'll do
- Maintain familiarity with the psychiatric caseload and nursing, medication, and continuity needs
- coordinate medication verification and continuity during transitions
- monitor medication-related needs and follow-up
- gather relevant records and clinical information
- support readiness for psychiatric evaluation
- serve as a nursing contact for routine psychiatric medication concerns
- coordinate treatment-plan follow-through
- liaise across clinical teams
- promote closed-loop communication and escalate unresolved issues.
