Job overview

The Registered Nurse (RN), Home Health role is onsite in Kalispell, MT, with pay of $35–$45 per hour. The nurse reports to the Home Health Director and provides care through Granite Peak Home Health. The role assesses patients’ nursing needs, develops and updates plans of care, and coordinates services to support patient health and continuity of care in the home health setting. Assigned duties include on-call coverage at night, on weekends, and on holidays.

What you'll do

• Provide services according to the plan of care
• conduct initial and ongoing nursing evaluations
• initiate and revise care plans
• provide specialized nursing and preventive or rehabilitative procedures
• document visits and care conferences
• coordinate services and communicate changes in patient condition
• counsel patients and families
• process orders and complete certification, recertification, discharge, and OASIS documentation
• refer patients to other professionals as appropriate
• supervise and teach nursing personnel
• and take assigned on-call duty nights, weekends, and holidays.
