What you'll do

• Conduct comprehensive in-home assessments
• develop and manage care plans with physicians and care teams
• provide skilled nursing, medication management, treatments, and interventions
• case-manage and coordinate therapists, aides, and physicians
• monitor progress and make clinical decisions
• educate patients and families
• coordinate transitions and discharge planning
• document in the EMR
• supervise and support Home Health Aides as needed
• participate in on-call rotation as appropriate.
