What you'll do

• Conduct and coordinate resident assessment development and completion
• maintain and update MDS and care-plan policies and procedures
• assist residents with the care-plan portion of discharge planning
• coordinate notification procedures for assessment team members about new admissions
• help schedule assessment and care-plan meetings
• assist with treatment and activity decisions based on residents’ medical and social histories
• participate in assessments and care plans, including quarterly and annual reviews.
