What you'll do
- Assess clients' physical, emotional, and social needs and develop individualized rehabilitation plans
- coordinate care with multidisciplinary teams
- supervise PRP workers providing therapeutic interventions
- educate clients and families about rehabilitation, coping strategies, and community resources
- maintain client documentation
- support staff service delivery and documentation
- assist with IRP review and compliance monitoring
- ensure monthly contact requirements
- complete progress notes within 24–48 hours
- participate in chart audits and supervision.
