Community Health Worker — Pierce County

What you'll do

• Engage prospective care management participants
• conduct community outreach, including home visits
• help people access transportation and community resources
• follow up by phone or in person to coordinate reminders, medication and medical appointments
• coordinate referrals with care managers
• provide status updates and document cases
• support socialization, recovery, self-advocacy and community living skills
• share health information to support treatment adherence. Field work and travel are required at least 50% of the time.
